RN Diabetes Care Manager
- Department: Diabetes Education
- Hours: 32.00 per week
- Shift: Shift 1
- No Weekends, No Holidays
The RN Diabetes Care Manager is competent with the knowledge and skills to create individualized diabetes self-management action plans, interpret clinical data, conduct focused and or complete educational assessments and coordinate care for the chronic care patient population across the health system. A Diabetes Care Manager will work with patients, families and the healthcare teams to improve outcomes and quality of life.
Minimum Qualifications:
Graduate of an accredited Bachelor of Nursing Program
Registered Nurse with an active license to practice in State of CT
Five (5) years of clinical practice
Preferred Qualifications:
Masters in Nursing or Enrolled/Interested in a program of attaining Masters.
Certified Diabetes Care and Education Specialist (CDCES) or experience in education. Or must begin the CDCES certification process within 6 months of hire.
Case/care coordination management experience.
