Position Summary
Ensures comprehensive and thoughtful connection to clinical and non-clinical supports for patients seeking outpatient, mental health care. Responsible for screening calls and providing appointments and mental health care information and referrals to patients.
Essential Duties & Responsibilities
- Responsible for engaging patients both on the phone and in-person that promotes effective triage and care coordination for patients seeking mental health care
- Screen callers requesting treatment for behavioral health needs, strengths, and coping skills
- Responsible for regular collaboration with clinical treatment coordinator to ensure patient access, throughput and connection to care
- Facilitate caller treatment appointments and perform follow up to ensure connection
- Assist with throughput, running reports and identifying patients that require follow up
- Maintain an awareness of area resources for appropriate referrals if needed
- Maintain accurate scheduling in EHR and ability to organize and provide data for insurance and credentialing
- Organize and prioritize workload in a fast paced environment
- Manage confidential matters with discretion and maintain a professional demeanor
- Ensures compliance with related federal, state and local regulations, as well as departmental policies and procedures.
Minimum Qualifications
- Bachelor’s level position
- Strong computer literacy skills
- Must have excellent writing and communication skills, be highly organized
- Must be able to work as a part of a team with patients, staff, and leadership
Preferred Qualifications
- BSW or human services background
- Experience working in mental health field
- Experience/knowledge of substance use disorders, autism, child development, and LGBTQ
