Job Summary
Provide individual and group services to adults impacted by a serious mental illness utilizing the evidence-based, internationally recognized Clubhouse Model and in coordination with DMAS criteria for psychosocial rehabilitation services. Complete clinical documentation ongoing. Share responsibilities for functions of the Clubhouse Program such as employment, education, housing, unit activities, and social/fundraising activities. Provide individualized services that focus on skill building. Engage, mentor, and work side-by-side with individuals in order to create an environment of belonging, with a focus on individual strengths, talents and interests. Assist individuals in developing vocational, educational, housing, wellness and social goals and objectives.
Job Responsibilities
- Complete daily clinical notes; collaboratively construct individualized service plans and quarterly reviews. Complete and submit insurance authorizations and assessments if appropriate.
- Coordinate, plan and participate in Clubhouse unit responsibilities to meet program and member needs; facilitate tasks and projects through involvement of members in meaningful work by maximizing members’ skills, talents, engagement and leadership.
- Establish meaningful relationships with members/individuals through side-by-side work to complete daily tasks; provide redirection when needed and maintain healthy boundaries. Provide vocational and educational assistance within a continuum of high and low intensity services to help members achieve self-defined success and to remove barriers.
- Assist and advocate for members/individuals within the community (i.e. housing, disability benefits, medical care, etc.).
- Offer support, assistance, consultation, and education to employers, relevant providers, family members/caregivers, and other involved parties when necessary to adequately support members/individuals and minimize barriers. Provide general educational to the community regarding program philosophy, related outcomes and services provided.
- Monitor, evaluate and record individual progress with respect to relevant treatment goals in an accurate and timely manner. Use electronic health records to document progress notes daily.
- Demonstrate fiscal responsibility by supporting agency collection policies and procedures; work proactively with payer sources to ensure payment of services; align services with payer authorizations and/or request authorization prior to provision of services.
- Meet guidelines and program requirements for fidelity and evidence-based Clubhouse model; ensure professional performance effectively and actively promotes the Standards for Clubhouse Model of Psychiatric Rehabilitation as developed by Clubhouse International.
- Provide training and support to new members/individuals, interns, volunteers, and other Clubhouse colleagues as necessary.
- Plan, participate in and facilitate fundraising, social/recreational activities, and special events. This includes some weekend and holiday activities.
- Facilitate daily living, mental health and wellness workshops/activities.
- Manage special projects as assigned by Program Coordinator.
- Perform other duties as assigned.
Job Requirements
Essential
- Must pass all required Background Checks.
- Must possess a valid Virginia Driver's License with reliable transportation and driving record acceptable to agency insurance carrier.
- Must obtain a TB test with satisfactory results within 30 days of employment, and yearly thereafter as required by licensing authority.
Education
Bachelor's Degree in Human Services, Social Work, Psychology, or related field; is certified QMHP-A within the Commonwealth of Virginia (QMHP-T considered).
Experience
- Experience with the Clubhouse Model, vocational services and mental health skill building preferred.
Preferred Competencies
Has considerable knowledge of the principles, practices, and techniques of psychosocial rehabilitation, crisis intervention, case management, and mental health issues. Has considerable knowledge of the literature, trends, and developments in the field of psychosocial rehabilitative services. Is skilled in promoting the integration of services for psychosocial rehabilitation among a variety of service agencies. Is skilled in assisting, coordinating, and supervising the activities of mentally ill individuals. Is able to use standard office and household machines, including telephone, calculator, copier, washer/dryer, stove, television, lawn equipment, microwave, etc. Is able to compile information from a variety of sources and prepare clear and concise reports. Is able to exercise initiative and independent judgment in applying standards to a variety of emergency and non-emergency situations. Is able to effectively express ideas orally and in writing. Is able to exercise tact, courtesy, firmness, and fairness in contact with individuals, supervisors, families, and the general public in the application of program regulations and guidelines. Is able to exercise considerable discretion in handling confidential files and cases. Is able to establish and maintain effective working relationships as necessitated by work assignments.
