Position Overview

Family Model Providers welcome adults with intellectual, developmental, or physical disabilities into their homes, providing a safe, supportive environment while promoting independence, dignity, and quality of life.
Key Responsibilities
- Provide a safe, clean, and welcoming home environment.
- Assist with daily living activities, personal care, and individual needs.
- Promote independence, community involvement, and meaningful relationships.
- Follow each individual's Person-Centered Support Plan (PCSP).
- Provide transportation to appointments and community activities.
- Maintain accurate documentation and communicate with Program Coordinators.
- Ensure individual health, safety, rights, and regulatory compliance.
- Must be at least 18 years old.
- Have a safe, stable home with appropriate accommodations.
- Maintain current Homeowners or Renters Insurance.
- Maintain current Automobile Insurance and reliable transportation.
- Complete all required Relias Training.
- Obtain and maintain Medication Administration Certification.
- Successfully complete required background checks and screenings.
- Complete required home inspections and ongoing monitoring.
- Maintain all required certifications and compliance documentation.
- Demonstrate compassion, dependability, and strong communication skills.
- Previous caregiving experience preferred but not required.
- Receive compensation for providing approved residential support.
- Make a meaningful difference from your own home.
- Help individuals achieve independence and personal goals.
- Receive ongoing training, guidance, and support.
