Purpose

This position coordinates reentry services and connects individuals to community-based resources to support the successful transition of youth following out of home placement.

Duties and Responsibilities

  • Coordinate reentry services for eligible youth by developing and implementing comprehensive care plans addressing housing, mental and physical health, substance abuse, education, employment, and other individualized needs.
  • Assess individual youth requirements and project needs through data collection and standardized assessments while prioritizing project tasks.
  • Execute project action plans and prepare required documentation including quarterly, annual, and special reports.
  • Facilitate effective coordination among cross-functional team members, community partners, social service agencies, housing providers, mental health providers, education systems, workforce development agencies, and substance abuse providers.
  • Participate in regular team and community partner meetings to ensure clear communication and coordinated service delivery.
  • Maintain accurate statistical records and generate reports to inform program improvements.
  • Engage in business development activities by analyzing requests for proposals, bids, and contracts in alignment with organizational objectives.
  • Establish and nurture strategic relationships with local community partners to secure resources and support successful youth reintegration.
  • Coordinate scheduling and communication for complex multi-participant meetings while resolving logistical challenges.
  • Collect and analyze industry and employment trends to drive innovation in reentry services.
  • Represent the best interests of MPHI at all times.
  • Additional duties as assigned.

Qualifications/Requirements

  • Bachelor's degree with 3-5 years of experience.
  • Master's degree in Sociology, Psychology, Counseling, Social Work, or a related field preferred.

  • Exhibit advanced academic and policy research skills and the ability to quickly assimilate new, complex information.
  • Communicate effectively both orally and in writing while fostering active relationships with community partners.
  • Plan, coordinate, and prioritize multiple tasks to manage complex reentry projects independently.
  • Leverage strong interpersonal and active listening skills to build collaborative relationships with diverse stakeholders.
  • Maintain cultural competency and sensitivity in interactions within varied community settings.
  • Ability to speak Spanish preferred.


Physical Demands

  • Constantly required to exchange accurate information.
  • Constantly operates a computer and other office machinery.
  • Constantly observes details at close range.
  • Frequently remains in a stationary position.
  • Occasionally moves about inside an office.
  • Occasionally moves office equipment weighing up to 25 pounds.

Work Environment

This job is performed in an indoor office environment and involves regular use of computers and other standard office equipment. The work environment may include limited exposure to moderate noise levels. Travel required in-state and out-of-state.
