Job Summary:
We are seeking a compassionate, organized, and resourceful Case Manager to join our team! Case Managers are at the core of our care coordination model—working directly with clients, providers, and community partners to ensure individuals are supported, empowered, and positioned for long-term success.
In this role, you’ll engage in frequent client communication, develop care plans, connect individuals with mental health and community resources, and provide health and psychoeducation. Your work will help advance our mission of making high-quality mental health care more accessible and inclusive for the individuals we serve.
Key Responsibilities:
- Conduct client outreach to assess needs, connect to resources, provide follow-up, and offer support
- Coordinate care among therapists, medical providers, and external agencies
- Link clients to essential resources, including housing, transportation, and food assistance
- Provide psychoeducation and guidance on navigating health systems and community supports
- Maintain timely and accurate documentation in our electronic medical record (EMR)
- Submit billing for reimbursable case management services
- Collaborate with the clinical team to support client goals and continuity of care
- Advocate for client needs while upholding ethical boundaries and confidentiality

Qualifications & Skills:
- Master’s Degree in Social Work
- Limited or Full Licensure (LLMSW, LMSW)
- Prior experience in mental health, social services, or case management strongly preferred
- Excellent interpersonal, organizational, and communication skills
- Familiarity with statewide and local resources
- Comfort with electronic documentation and EMR platforms
- Deep commitment to client-centered care and equitable access to services
