Care Management Registered Nurse (RN)

Position Summary
The Care Management RN collaborates with patients, providers, families, and community resources to coordinate care for patients with chronic and complex medical conditions. This role focuses on patient education, care coordination, and self-management support to improve health outcomes and overall quality of life.
This is a Monday- Friday Position. No weekends or holidays.
Key Responsibilities
- Support MPCP’s mission of providing accessible, high-quality healthcare services.
- Build and maintain relationships with community agencies and maintain a resource library addressing social determinants of health.
- Assess patient needs and develop individualized, patient-centered care plans and goals.
- Document all care management activities and care plan progress within the electronic health record (EHR).
- Monitor patient adherence to care plans, evaluate effectiveness, and facilitate timely adjustments.
- Support shared decision-making with patients and families regarding care coordination needs.
- Coordinate access to appropriate medical and specialty services, including facilitating smooth inpatient-to-outpatient transitions to reduce readmissions.
- Collaborate closely with providers and staff regarding symptom management, medication oversight, and patient and family education.
- Promote self-management and healthy lifestyle changes through coaching and behavior modification interventions.
- Attend and participate in staff meetings and contribute to ongoing improvement initiatives.
- Perform other duties consistent with MPCP’s mission as assigned.
Requirements & Qualifications
- RN degree from an accredited nursing program required.
- Minimum of 1 year clinical nursing and/or case management or managed care experience.
- Preferred experience in a physician office or outpatient clinic setting.
Skills & Abilities
- Strong knowledge of community resources.
- Excellent organizational, communication, and customer service skills.
- Ability to work effectively with individuals from diverse backgrounds.
- Demonstrated ability to work independently, exercise sound judgment, and function as part of a team.
- Proficient in EHR systems and computer applications.
- Commitment to patient confidentiality and adherence to HIPAA standards.
Other Requirements
- Willingness to travel between MPCP locations as needed (Big Rapids, Reed City, and Canadian Lakes/Stanwood).
