Job Summary
We are seeking a dedicated, compassionate, and proactive Community Liaison to serve as a vital connection between Michigan Palliative and Hospice Care and the healthcare community. The ideal candidate will be responsible for developing and maintaining strong relationships with community stakeholders, promoting awareness of our services, and enhancing our visibility through strategic outreach and communication efforts.

This role requires a blend of marketing expertise, exceptional interpersonal skills, and a genuine passion for community engagement and patient-centered care.

This vital role supports our commitment to patients and their families by promoting palliative and hospice services with empathy and professionalism throughout the healthcare community.

In this role, you will actively promote awareness of Michigan Palliative and Hospice Care services through relationship-building, community outreach, and strategic marketing initiatives across Wayne, Oakland, and Macomb counties. You will cultivate meaningful partnerships with physicians, hospitals, nursing facilities, assisted living communities, and other healthcare organizations to support referrals and admissions to our services.

This is a remote position with the expectation of daily local travel for in-person meetings, presentations, and relationship-building within physician offices, hospitals, and other healthcare facilities. Responsibilities include educating referral sources on palliative and hospice services, clarifying referral processes, delivering presentations, and fostering trusted partnerships that support quality patient care and service growth.

Responsibilities:

  • Cultivate referrals from physicians, hospitals, nursing facilities, and other healthcare and community organizations through the education of palliative and hospice services.
  • Deepen relationships with current and new physicians, hospitals and facilities.
  • Meet or exceed referral and admission targets.
  • Continuously expand your knowledge of palliative and hospice care to become an expert, enabling you to effectively educate and support our community.
  • Maintain standards of high-quality customer service, and show respect to all constituents, both internal and external.
  • Implement marketing and promotional initiatives as directed by the leadership team.
  • Monitor and report on cost-effectiveness of marketing efforts.
  • Deliver educational training and information sessions to patients, families, physician offices and hospital staff, becoming a trusted resource for our patients, healthcare professionals and key community partners on palliative and hospice care.
  • Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees, and other related activities as needed.
  • Share insights on each referral source's preferences for communication, ensuring their protocols and procedures are followed closely.

Requirements

  • Associates or Bachelor’s degree in healthcare administration, marketing, business, social work, nursing or a related field is preferred. Relevant experience in healthcare outreach, community relations, or marketing may be considered in lieu of a degree.
  • Demonstrated history of positive referral/business growth results, preferably involving sophisticated and meaningful stakeholder relationships that led to outstanding results.
  • Valid Driver's License, proof of auto insurance coverage. Access to a reliable vehicle suitable for daily travel to healthcare facilities, physician offices, and community organizations.
  • Proven experience in community outreach or a related field.
  • Excellent communication, presentation, marketing negotiation, and public relations skills.
  • Ability to market effectively through presentations, written material and other marketing tools.
  • Strong skills in analytics to evaluate outreach effectiveness.
  • Experience in hospice and palliative care preferred.
  • Exceptional communication and interpersonal skills to engage effectively with diverse audiences.
  • Ability to work independently as well as collaboratively within a team environment.
  • Familiarity with palliative and hospice care services is an asset, along with a willingness to learn and stay informed about service trends.
