Responsibilities*
Associates degree in a related field or an equivalent combination of education and experience
Three years of demonstrated and specific experience working with older adults with mild to moderate dementia and their family/caregivers
Physical Requirements
This position requires frequent standing/walking, as well as lifting, positioning, pushing, and/or transferring of participants.

- Collaborate with the care team to plan, implement, and evaluate activity care programs
- Plan and conduct appropriate activity programs for patients
- Evaluate and document patients' activities, progress and the emotional, social, mental and/or physical condition of patients through the activity.
- Organize and maintain program materials, supplies, and equipment
- Communicate with family members regarding participant's status
- Monitor participants continued eligibility for program
- Participate in staff meetings and committees as assigned
- Ability to develop educational programs on a specific topic
- Perform other duties as assigned
