Job Description
Provides direct patient care within scope of practice, as deemed necessary by the physician and the physical therapist, in accordance with the established care plan that can be carried out in a safe manner in the patient’s home
Assist the Physical Therapist and Physician in the evaluation of the patient’s current functional status and overall abilities to help determine realistic outcomes
Delivers a variety of Physical Therapy treatments in accordance with the plan of care and accepted standards of treatment specific to patients of various Physical Therapy diagnoses
Provides care based on patient and family needs, identifies goals, interventions and expected outcomes with patient/caregiver/family involvement
Observes and documents treatments administered and patient reaction to treatments given
Contributes to evaluation and planning process that addresses physical, psychosocial, environmental and safety factors while considering physician orders, relevant history, patient’s functional status, and availability of willing and able caregiver
Evaluates subjective complaints and objective changes in patient’s status and progress
Evaluates, reports and records patient's status, changes and progress to the Physical Therapist, physician, and the health care team
Uses appropriate intervention to meet physical, psychosocial, environmental and safety needs of the patient
Completes necessary clinical notes legibly, accurately, and timely
Documents patient limitations and progress, and submits paperwork in a timely manner
Provides patient/caregiver education with a focus on goal and outcome achievement as established in the plan of care, documenting patient response to teaching in clinical notes
Uses teaching methods as appropriate, including demonstration, verbal instruction, and written material
Graduate of an accredited physical therapy assistant program
Current certification or license as required by state law
Possesses and maintains current CPR certification
Job Type: Full-time
Pay: $55.00 - $65.00 per hour
