Key Responsibilities
- Perform comprehensive patient assessments and ongoing evaluations
- Develop, implement, and revise individualized care plans
- Administer medications, injections, and IV therapies as prescribed
- Provide wound care and monitor healing progress
- Monitor and manage chronic conditions (e.g., diabetes, heart failure, COPD)
- Educate patients and family members on disease management, medications, and safety
- Document all patient care accurately in electronic health records (EHR)
- Communicate changes in patient condition to physicians and care teams
- Coordinate care with physical therapists, occupational therapists, social workers, and other healthcare professionals
- Ensure compliance with agency policies, state regulations, and federal guidelines
- Participate in case conferences and quality improvement activities
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- Graduate of an accredited nursing program
- Current BLS/CPR certification
- Minimum of 1–2 years of clinical nursing experience (home health experience preferred)
- Valid driver’s license and reliable transportation
- Ability to work independently and manage time effectively
Preferred Qualifications
- Experience in home health, hospice, geriatrics, or medical-surgical nursing
- Strong wound care and IV therapy skills
- Knowledge of Medicare/Medicaid regulations
- OASIS documentation experience
