Key Responsibilities
- Navigate and coordinate care across healthcare systems
- Identify and prevent potential medical complications
- Develop and implement individualized care plans
- Support patients with chronic and complex conditions
- Collaborate with providers, families, and interdisciplinary care teams
- Maintain accurate documentation and ensure compliance with healthcare regulations
- Active and unrestricted RN license in the state of Michigan
- Minimum of 1–2 years of clinical experience (case management, home health, rehab, or acute care preferred)
- Strong understanding of care coordination, discharge planning, and chronic disease management
- Knowledge of healthcare regulations, documentation standards, and compliance requirements
- Experience with EMR/EHR systems
- Excellent clinical judgment and critical thinking skills
- Strong communication and patient education abilities
- Ability to work independently, prioritize tasks, and manage a flexible schedule
- Valid driver’s license and reliable transportation
