Duties
- Implement and monitor personalized care plans designed to restore and maintain residents’ physical abilities and functional independence.
- Assist residents with activities of daily living, including mobility, hygiene, grooming, and nutrition, ensuring dignity and comfort at all times.
- Conduct regular patient monitoring to assess progress, document changes in condition, and communicate updates to the nursing team.
- Review medical records and documentation to ensure accuracy, compliance with HIPAA regulations, and proper case management.
- Utilize EMR (Electronic Medical Records) systems to document care activities, update resident information, and review medical histories efficiently.
- Support nurse management by supervising shift activities, managing schedules, and ensuring smooth workflow during your assigned hours.
- Collaborate with interdisciplinary teams to develop care plans aligned with Medicare guidelines and resident’s needs.
- Assist in training new staff or aides on restorative techniques, documentation review procedures, and safety protocols.
- Maintain compliance with healthcare regulations including HIPAA standards while safeguarding patient confidentiality.
- Contribute to budgeting efforts by managing supplies efficiently and advocating for resources that enhance resident care.
Experience
- Prior experience in critical care settings such as assisted living facilities, home health agencies, hospice care environments, or acute care hospitals is highly desirable.
- Demonstrated supervising or management experience within nursing or healthcare teams is preferred.
- Familiarity with EMR systems for medical records management and documentation review is essential.
- Strong knowledge of physiology, anatomy, medical terminology, and care planning principles underpin effective restorative nursing practices.
- Proven ability to monitor patient conditions accurately through vital signs assessment and resident observation techniques.
- Leadership skills demonstrated through shift management responsibilities or supervising other healthcare aides are advantageous.
