Job Title : Physical Therapist MJH PRN
Job Summary
Evaluates and implements physical therapy treatment plans to patients with physical impairments related to the musculoskeletal, neurological, integumentary system, and other conditions that may impact physical function. Physical therapy works to restore and / or maintain the patient's function, alleviate pain, and prevent disabilities by planning and administering medically prescribed skilled physical therapy interventions. Collaborates with physician, family members and other members of the healthcare team. Provides training and education to family / caregivers and other professionals.
Essential Job Functions
- Completes a comprehensive physical therapy evaluation and plan of care
- Develops a comprehensive treatment plan with goals that align with identified impairments for each participant to maximize potential
- Begins discharge planning on evaluation and involves the patient, family and / or caregiver in establishing goals for rehabilitation and discharge planning, as evidenced in the documentation
- Reviews patient's status at regular intervals and changes the treatment program as necessary
- Participates in and provides in-service education to facilities, supportive staff, students, and the community
- Attends both departmental and IDT (interdisciplinary team) meetings
- Establishes a cooperative working relationship between the IDT, rehab team and members of the medical community
- Supervises and trains support personnel
- Documentation aligns with best practice standards and is accurate and timely
- Identifies potential problem situations, intervenes appropriately, and communicates such events to supervisor
- Performs related non-clinical services necessary to maintain the efficient operation of the department, and adjusts schedule as required
- Demonstrates willingness to work across clinical service lines, including inpatient, outpatient, and PACE as needed
