What are the responsibilities and job description for the Rural Intensive Case Manager (TCOOMMI) position at MHMR Authority of Brazos Valley?
Job Description: To perform Intensive Case Management activities for 20-25 adults receiving both MHMRABV mental health services AND specialized community supervision services through Adult Probation and Parole in the rural counties. The TCOOMMI Intensive Case Manager's objective is to assist justice involved clients in increasing independent functioning in the community, reducing recidivism in the criminal justice system, reduce hospitalizations, and developing individualized short and long term recovery goals. Job functions include, but not limited to completing ANSA assessment, developing individualized Recovery Plans, performing skills training, intervening in crisis situations, coordinating community resources and documenting all contacts for persons served on caseload. This position works closely with Special Needs Probation and Parole staff, following Program Guidelines and Procedures set forth by TCOOMMI, to monitor client's mental health treatment and legal requirements.
Education: Requires Bachelor’s Degree in a Behavioral, Social Science, Humanities or Education field.
Experience: Requires experience working with individuals with mental illness, must be able to work independently as well as with a team. Experience working in a Criminal Justice field is preferred. Good organizational skills are a must.
Hours of Work: Generally Monday—Friday 8am-5pm; adjustments in work hours maybe warranted to meet the needs of the program. Crisis on Call work required as agency need dictates.
Licensure/Certification: Ability to be credentialed as Qualified Mental Health Professional. Valid driver’s license and are insurable by agency insurance. Continued employment is contingent upon successful completion of Adult Needs and Strengths Assessment (ANSA) certification as well as Illness, Management and Recovery (IMR) certification.
Physical Requirements: Ability to sit or stand for extended periods of time, communicate effectively both verbally & written, good computer skills a must; ability to lift/carry under 30lbs; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
