What are the responsibilities and job description for the MAPs Coach position at MHDS?
BRIEF JOB SUMMARY: MAPs serves people with intellectual and developmental disabilities either exiting the school system or who have recently exited and may be waiting for traditional waiver services. The MAPs program will provide services that encourage sustainable independence and focus on:
· Identifying people and places in their local communities that share like interests or align with their future goals.
· Learning to harness the power of accessible and affordable technologies to foster their own functional independence.
· How to navigate and travel independently in their community and how to build natural support networks
· Independent living skills
MAPs is a grant funded program through the Department of Intellectual & Developmental Disabilities for 3 years. This position would market, oversee activities, perform duties for services and serve as the liaison between the agency, person served and department.
ESSENTIAL JOB FUNCTIONS:
1.The Provider Supports work with individuals, their families, and other people in their support network with the aim of helping the person to identify and make connections with their chosen activities, resources, and interests, to develop their Virtual Community Resource Map (VCRM).
2. The VCRM is a fully customized, online interactive resource map that is used by a person supported to organize resources, assist with person-centered planning, and complete travel training experiences for the person. The digital map is a resource, but it is also an instructional environment where travelers can:
a. Increase individuals' geographical awareness and knowledge of what is available to them within their community.
b. Build their daily schedules (school, work, etc.) into the map with customized text descriptions and pictures.
c. Learn how to travel in the community.
d. Access supports and services.
e. Participate in social and recreational activities with peers.
3. The VCRM teaches and guides the planning, exploration, and attainment of community membership. Community Navigator services can be used in support of the VCRM and are focused on helping the person
develop community awareness and social connections that are the foundation of work outcomes, competitive integrated employment, self-employment, and/or participation in post-secondary education opportunities.
4. Assisting someone in their identification and pursuit of their milestones, outcomes, interests and/or goals that are unique to each person, being assisted to locate the opportunities in their local communities that are relevant to each person’s unique interests and goals, and to identify other people with similar interests and goals are necessary for the creation of the VCRM.
5. Collaborative Goal Setting: Involve the MAPS participant and their natural support(s) in developing goals regarding the eleven possible milestones offered by MAPS program, by participating in Person Supported Collaborative Plan (PSCP) meeting.
6. Connecting MAPS participants to resources by identifying the individuals need and connecting them with appropriate services such as housing, employment, healthcare, or education. This may involve MAPS Coaches participating in community engagement and/or leveraging local networks.
7. Participate in all training, informational sessions, surveys, and workshops regarding the MAPs Program.
8. Bill for the services provided through the MAPs Program.
9. Complete all referral and outcome-based activities in a timely and complete manner. Maintaining records of activities.
10. Participate and facilitate in the marketing of activities for the MAPs program on behalf of MHDS, Inc.
11. Be knowledge of technology solutions to promote independence.
