Overview
We are seeking a dedicated Chronic Care Management Specialist to join our team. They will be responsible for coordinating patient care and providing support to individuals in need of Chronic Care Management.
Duties
Calling patients monthly
Building Care Plans to help high-risk patients reach monthly goals
Communicating between the patient and provider, including any specialty providers
Document communication with patients, the patient’s caregivers, and specialty providers in the EMR
Following CMS guidelines for chronic care management
Reviewing labs, radiology reports, specialty provider notes
Scheduling appointments, checking referral status
Qualifications
- Experience in healthcare, preferably in a clinical setting
- Proficiency in medical records management
- Knowledge of medical terminology and procedures
- Strong communication and interpersonal skills
-Strong clinical background, care plan coordinator, or previous CCM experience
