Role Description The Advanced Practice Provider (APP) will provide on-site clinical care in subacute rehabilitation settings in Edison, NJ, in a part-time capacity. Day-to-day responsibilities include performing patient assessments, managing acute and chronic medical conditions, and collaborating with attending physiatrists and facility staff to develop and adjust rehabilitation-focused care plans. The APP will review diagnostic data, prescribe medications, order appropriate therapies, and monitor progress toward functional and medical goals. Additional duties include documentation of encounters, participation in interdisciplinary team meetings, and communication with patients, families, and facility staff to ensure coordinated, high-quality care. The role involves practicing evidence-based medicine while supporting safe transitions and continuity of care.
Qualifications
- Current licensure and certification as a Nurse Practitioner or Physician Assistant (Advanced Practice Provider), with eligibility to practice in New Jersey.
- Strong assessment, diagnostic, and clinical decision-making skills, including the ability to interpret labs, imaging, and consult recommendations.
- Effective communication and collaboration skills for working with physicians, nursing, therapy services, patients, and families in an interdisciplinary environment.
- Commitment to patient-centered, function-focused care, and comfort working in subacute rehabilitation or similar post-acute care settings.
- Graduate degree from an accredited NP or PA program; board certification in a relevant specialty (e.g., Adult-Gerontology, Family, Internal Medicine) preferred.
- Prior experience in PM&R, post-acute care, skilled nursing facilities, or inpatient rehabilitation is beneficial but not required.
