Schedule: Full Time (40 hours per week)
Location: Springfield, MA and surrounding areas
Responsibilities:
- The Case Manager is responsible for visiting clients in the home (currently done through TeleHealth) to coordinate and inspect their well being.
- Coordinate with Program Director and Nurse to ensure participant satisfaction
- Participating in the development, implementation and ongoing review of the plan of care.
- Completing a case-management progress note corresponding with each on-site visit or encounter and upon significant change.
- Assisting with obtaining information and accessing other health-care and community services
- Reporting changes in member's condition to the registered nurse
- Assisting with making referrals to appropriate service providers if the member requires services other than those provided by our programs
- Providing timely assistance and responding to urgent or emergency needs of the member
- Other functions as directed
Requirements:
- Bachelor's Degree
- Excellent communication skills
- Prior stable work experience
- Familiarity with computer and Windows PC applications
- Ability to multi-task, this includes ability to understand multiple products and multiple levels of benefits within each product
- Ability to remain focused and productive each day though tasks may be repetitive
