Responsibilities:
RESPONSIBILITIES/ACCOUNTABILITIES:
1. Tracks all new and incoming evaluations and requests for services.
2. Gathers patient information from parents, counties, and/or districts for all incoming referrals.
3. Assigns cases to the appropriate therapy team (Occupational Therapist, Physical Therapist, Speech
Language Pathologist, Social Worker, etc.).
4. Communicates needs for additional providers or recruiting efforts to department leadership.
5. Communicates with parents, teachers, medical professionals, and other stakeholders as needed to
facilitate collaboration and continuity of care.
6. Reviews evaluations and uploads completed evaluations into the child’s record.
7. Sends final copies of assessments and evaluations to appropriate parties.
8. Maintains complete and accurate records for all students and clients.
9. Assists with preparation for audits, as needed.
10. Works collaboratively with the billing department to resolve any issues with session notes, claims, or
invoices.
11. Performs other related duties as assigned.

3. Promptly reports concerns and suspected incidences of non-compliance to supervisor, Compliance
Liaison or to the Compliance Officer via the Integrity Hotline.
4. Cooperates with monitoring and audit functions and investigations.
5. Participates, as requested, in quality assurance and process improvement activities.
Posted Salary Range: USD $21.00 - USD $25.00 /Hr.

The Services and Evaluation Coordinator assigns service providers to clients and
students who are receiving therapy or educational services from Metro Therapy. They coordinate with
staff to ensure services or evaluations are performed as required in the child’s IEP or IFSP, depending on
the child’s age and the program they are enrolled in.

SPECIFIC EDUCATIONAL/VOCATIONAL REQUIREMENTS:
1. A Bachelor’s degree in education or a related field or equivalent experience is required.
2. Must have 1 or more years of experience working in healthcare, education, or childcare
