As a Charge Nurse LPN or RN, you’ll combine clinical expertise with team coordination to ensure that each resident receives high-quality, person-centered care in a supportive community setting. What You’ll Do
- Lead and supervise the Neighborhood Care Team to deliver quality personal and clinical care to residents.
- Administer medications and treatments accurately and in accordance with physician orders and facility policies.
- Assess and monitor residents’ health status; promptly report changes and collaborate with physicians and families as needed.
- Coordinate and delegate daily assignments to care for team members, ensuring efficiency and resident satisfaction.
- Foster teamwork and mentorship, providing guidance and support to CNAs, PCAs, and other team members.
- Promote a person-centered approach, ensuring dignity, choice, and respect for each resident’s needs and preferences.
- Ensure compliance with all federal, state, and local regulations, as well as HIPAA Privacy and Security standards.
- Collaborate with interdisciplinary team members to develop, implement, and evaluate individualized care plans.
- Work a flexible schedule, including weekends and holidays as needed.
- Assume accountability for compliance with federal, state and local regulations within the assigned duties and within his or her span of control
- Will assume responsibility to ensure secure handling of information, and will adhere to HIPAA Privacy and Security standards
- Perform additional duties as assigned by leadership
What Else You Need
- Graduate of an approved LPN program (RN candidates also considered).
- Current, unencumbered LPN license in Pennsylvania.
- Minimum 2 years of nursing experience; at least 1 year in gerontology or long-term care preferred.
