Schedule: Monday–Friday, 10:00 a.m.–6:30 p.m. | No weekends
We are seeking a Transitions of Care (Admissions) Nursing Supervisor to lead nursing services during assigned shifts and help ensure high-quality, safe, and seamless care for residents. This role supports admissions, discharges, transfers, medication reviews, staff development, and day-to-day clinical operations.
What You’ll Do:
- Assist with rounding, resident assessments, and care coordination.
- Support smooth admissions, discharges, and transfers.
- Provide clinical leadership and support during assigned shifts.
- Conduct medication reviews and provide clear instructions to residents and representatives.
- Guide, mentor, and evaluate nursing team members.
- Assist with hiring, onboarding, orientation, and staff development.
- Collaborate with the interdisciplinary team to promote continuity and quality of care.

Qualifications:What We’re Looking For
- Active RN license in Pennsylvania.
- At least 3 years of nursing experience, including 1 year in gerontology.
- At least 1 year of supervisory experience.
- CPR certification.
- Bachelor’s degree preferred, but not required.
- Strong communication, organization, technology, and customer service skills.
Salary : $40 - $48
