What are the responsibilities and job description for the Case Manager RN - PRN (As Needed) position at Mesa View Regional?
The purpose of the Nurse Case Manager role is to provide consistent nursing leadership for interdisciplinary bedside rounding, care coordination, ensuring timely, patient-centered, and well-organized transitions of care for hospitalized patients. This position serves as the central point of coordination between hospitalists, patients, nursing, case management, and ancillary services to proactively identify and remove barriers to discharge, support patient flow, and enhance communication across the care team.
The Nurse Case Manager supports regulatory compliance, improves discharge efficiency, promotes patient engagement, and contributes to reduced length of stay, improved patient experience, and safe continuity of care across the healthcare continuum.
Education: Associate Degree Required. BSN preferred.
Experience: Prior experience as a Case Manager preferred. 3-5 years of nursing experience preferred.
Licenses/Certificates: Current NV RN License. Current BLS required.
