Job Overview
We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. This role involves providing therapeutic services, conducting assessments, and developing treatment plans tailored to the needs of clients. Responsibilities
- Perform Home Health patient assessments and diagnostic evaluations to determine appropriate treatment strategies.
- Develop and implement treatment plans based on clinical research and best practices in psychotherapy.
- Provide crisis intervention and management for clients in acute distress.
- Collaborate with multidisciplinary teams to coordinate care for patients with developmental disabilities, autism, or other special needs.
- Maintain accurate medical documentation, progress notes, and treatment plans in compliance with HIPAA regulations.
- Utilize EMR systems for data collection and management of patient records.
- Engage in case management activities to support clients' access to community resources and services.
- Participate in ongoing professional development to stay current with trends in social work, mental health counseling, and related fields.
Skills
- Proficient in various therapeutic modalities including cognitive behavioral therapy (CBT), psychodynamic therapy, and motivational interviewing.
- Experience with child & family counseling as well as pediatrics.
- Knowledge of medical terminology, CPT coding, ICD-9, and ICD-10 coding systems.
- Familiarity with home & community care practices and geriatric care.
- Strong understanding of behavioral therapy techniques and applied behavior analysis (ABA).
- Ability to conduct clinical research and contribute to data collection efforts.
- Experience working in hospital settings or providing crisis intervention services.
- Excellent communication skills for effective patient care and collaboration with healthcare teams.
