Description
The Monitoring Specialist works cooperatively with Directors and Supervisors in the Department of Personal Supports and Opportunities to conduct monitoring activities in host home settings. Monitoring activities include face-to-face monitoring and review of documentation. The Monitoring Specialist supports host home providers in resolution of problems found while monitoring, including providing applicable training as the situation may dictate. The Monitoring Specialist is responsible for facilitation of the processes that commence with a person-centered assessment and discovery and culminates in the development of person-centered supports that facilitate the individual in achieving his or her chosen goals. Completes assessments, conducts Natural Support Mapping and develops support plans for everyone on his/her caseload. The caseload will include individuals supported in host homes.
Requirements
- Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
- Oversees Host Home contracted employees including support to individuals and host home providers.
- Completes required organization forms, including, but not limited to HR forms, Payroll forms, Finance and Accounting forms, etc., and submits to appropriate department in an accurate and timely manner.
- Ensure all Host Home Providers are up to date and accurate for all. Trains host home providers on how to document, navigate our database, and on any necessary documents required for the individual.
- Attending meetings, in-services, training, etc., on a consistent basis as appropriate and directed by the Residential Director and Host Home Manager.
- Assures open communication with the guardian and other team members is completed. Addresses any concerns the guardian has in a timely manner, and documents follow-up.
- Assures all departments (i.e.: nursing, vocational, behavior, etc.) are working together to make sure all individual needs are met. Ensure Host Home Monitors have completed and followed up all necessary paperwork.
- Follows up on emails, phone calls, concerns, etc., in a timely manner, preferably within two business days.
- Reviews and responses to relevant T-Logs, General Event Reports, MANE’s, and/or other investigations in a timely manner.
- Promotes and fosters a supportive and person-centered atmosphere for individuals that reside in Strives Host Homes and our Family Care Giver settings.
- Responds to emergency situations as appropriate in an accurate and timely manner.
- Supports Strive providers and employees to ensure that individuals have opportunities to exercise their choice and pursue their own personal goals.
- Ensures that individuals are involved in all aspects of their daily routine in their homes.
- Participates in On-Call Rotation as assigned.
- Assures that individuals receive all Services and Supports identified in their personal Service Plans, and that required documentation is being followed correctly. Including implementation of Individual Service and Support Plans, Behavior Support Plans, Nursing Care Plans and Protocols, Therapy Plans and Protocols, and other needs as identified in the individuals Annual Service Plan.
- Participates in the development and implementation of new program initiatives.
- Facilitate screening and interviewing for Family Care Givers, Monitoring Specialist, Coordinator, and PCE employees, and documents accordingly. Routinely Monitors job performance and provides feedback, coaching, and offers support. Responds to personnel issues in a timely manner.
- Routinely review monitoring and solicit feedback from host home providers and provides coaching and support. Maintains and open communication with providers and follows up on concerns/recommendations resulting from monitoring activities.
- Ensure that HUD inspections are completed for each prospective host home or FCG setting. Follow-up to assure that any deficiencies noted in HUD inspection are corrected to prior initiation of contact. Ensures that the home meets the needs of the individual, including but not limited to safety and accessibility.
- Ensure that all new host home providers have established a trade name, obtained liability insurance, and completed other pre-contract requirements. Ensure that all Family Care Givers and Host Home Providers maintain updated insurance requirements and contractual agreements. Develops an understanding of the contract and monitors contract compliance.
- Establishes and maintains monitoring system to ensure that the individual receives all services and supports required by their service plans and by applicable regulations. Ensures that monitoring visits occur in accordance with the individual’s monitoring plan, including periodic unannounced visits. Reviews all monitoring visits documentation completed by the monitoring specialist and immediately addresses noted concerns. Conducts monitoring visits and completes all required documentation.

Experience:
- Minimum of 2 years paid work experience with individuals with disabilities.
