Community Navigator – Outpatient Care Management

As a Community Navigator, you’ll play a key role in supporting patients and families as they move through the healthcare system. You’ll be part of an outpatient care management team dedicated to improving health outcomes through compassionate support, community outreach, and coordinated care.

You’ll be a trusted guide for patients who need support navigating complex systems. Your work helps ensure that every patient receives coordinated, continuous care—improving their health, stability, and overall experience.

What You’ll Do
- Assist patients with completing applications, registration forms, and accessing community resources
- Help patients connect with transportation services and provide appointment reminders
- Support seamless care coordination from initial contact through case closure
- Provide general office and clerical support to the outpatient care management team
- Help patients navigate the healthcare system with confidence and clarity
- Contribute to population health goals through outreach and patient engagement
- Education: High school diploma or equivalent required; Associate Degree preferred
- Experience:
- Clerical experience in a healthcare setting preferred
- Familiarity with Microsoft Office, fax systems, and electronic medical records preferred
- Skills & Abilities:
- Strong communication and computer skills
- Knowledge of community resources (preferred)
- Basic understanding of medical terminology
- Ability to prioritize tasks, stay organized, and manage multiple responsibilities
