Role Description This full-time, on-site Licensed Clinical Social Worker role is based in Sewell, NJ. The LCSW will provide direct clinical services, including psychosocial assessments, individual and group therapy, and ongoing treatment planning for patients in a medical and behavioral health setting. Day-to-day responsibilities include coordinating case management services, collaborating with care team members, and advocating for patient needs and resources. The role involves maintaining accurate clinical documentation, participating in interdisciplinary care meetings, and supporting crisis intervention and safety planning as needed. The LCSW will also contribute to program development, quality improvement initiatives, and ensure compliance with ethical and regulatory standards.
Qualifications
- Strong foundation in Social Work practice, including person-centered care, advocacy, and resource coordination.
- Experience in Medical Social Work, integrating psychosocial care within medical treatment plans and collaborating with healthcare teams.
- Proficiency in Case Management, including care coordination, discharge planning, community referrals, and follow-up support.
- Clinical expertise in Mental Health and Behavioral Health, including assessment, diagnosis within scope, and evidence-informed therapeutic interventions.
- Active Licensed Clinical Social Worker (LCSW) credential in New Jersey, or eligibility to obtain licensure within state requirements.
- Master’s degree in Social Work (MSW) from an accredited program.
- Demonstrated ability to work with diverse populations, maintain professional boundaries, and uphold confidentiality and ethical standards.
- Strong communication, documentation, and interdisciplinary collaboration skills; experience in outpatient or treatment center settings is beneficial.
