Job Overview
We are seeking a highly motivated and detail-oriented MDS Coordinator to join our dynamic healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation of patient care needs within skilled nursing, rehabilitation, or long-term care settings. Your expertise will ensure accurate completion of Minimum Data Set (MDS) assessments, facilitating optimal patient care, regulatory compliance, and reimbursement processes. This position offers an exciting opportunity to impact patient outcomes through meticulous clinical documentation and interdisciplinary collaboration.

Duties

  • Lead the coordination and completion of MDS assessments in accordance with federal and state regulations, including CMS guidelines.
  • Collaborate closely with nursing staff, physicians, social workers, and other healthcare professionals to gather comprehensive clinical information.
  • Review medical records, clinical documentation, and care plans to ensure accuracy and completeness for each assessment.
  • Ensure timely submission of MDS assessments within mandated deadlines to support proper reimbursement and quality reporting.
  • Conduct ongoing education for staff on MDS updates, coding requirements, and regulatory changes related to the Resident Assessment Instrument (RAI).

Skills

  • Extensive knowledge of MDS 3.0 process, including accurate coding using ICD-10/ICD-9 codes and CPT coding for procedures.
  • Proficiency in managing care documentation within PCC (PointClickCare).
  • Strong understanding of Medicare regulations, NCQA standards, and utilization management protocols.
  • Experience with hospital settings, skilled nursing facilities, or home care environments; ICU or Level I/II trauma center experience is a plus.
  • Familiarity with clinical terminology, anatomy, physiology, and medical records review to support precise assessments.
  • Ability to interpret complex medical documentation and ensure compliance with HIPAA standards.
  • Excellent communication skills for interdisciplinary collaboration and staff education on documentation best practices. Pay: $70,000.00 - $90,000.00 per year
