Key Responsibilities:
- Evaluate member needs, barriers to care, and social determinants of health, prioritizing their identified goals and recommending tailored plans for optimal outcomes
- Develop ongoing care plans and collaborate with providers to identify necessary specialists, resources, and services to address member's unmet needs
- Identify and resolve problems/barriers to care, providing targeted interventions to ensure seamless care delivery
- Coordinate between members, caregivers, and care providers to guarantee person-centered care or services
- Provide ongoing follow-up and monitoring of member status, collaborating with them and appropriate providers to revise or update care plans as needed
- Offer resource support to members and care managers on local services such as employment, housing, independent living, and justice
- Facilitate timely access to necessary care or services through collaboration with providers and specialists
- Perform telephonic, digital, home, and/or site outreach to assess member needs and collaborate with resources
- Collect, document, and maintain member information and care management activities to ensure compliance with regulators
- Provide education to members and their families/caregivers on disease processes, care options, referrals, and benefits
- Share feedback with leadership on opportunities to improve and enhance care delivery in a cost-effective manner
Requirements:
- Bachelor's degree in Nursing or related field, with 2-4 years of experience in care management
- RN licensure and/or Compact State Licensure required
