About the Role

As the CCBHC-SOAR Specialist you will play a pivotal role by completing applications for benefits for those clients who are uninsured and meet qualifications to obtain these benefits. The SOAR Specialist will broker services and provide advocacy services for adults with SMI and other mental health issues, substance use or co-occurring disorders, including opioid disorder client in Meridian's CCBHC. The SOAR Specialist will coordinate the care for consumers, and work with other professionals, including Primary Care Clinic, and other healthcare, community organizations, as well as with Meridian staff to assist clients with obtaining benefits.

You will:

• Perform clinical assessments to determine diagnosis, level of care, and treatment plan in coordination with the Team.

• Provide therapy services to assigned clients and/or family members in accordance with Center guidelines and procedures, and best practice standards for profession.

• Follow the appropriate evidence-based model as provided.

• Provide crisis intervention services for clients as needed.

• Conduct face-to-face visits with clients in their home or in the community to monitor progress.

• Have availability between the hours of 8 am and 8 pm daily.

• Travel to the home of clients or community settings.

• Provide on-call duties as needed.

What We're Looking For

• BA/BS from an accredited university with major course work in the areas of health related/ psychology/social work areas with at least 1-year experience working with SSI/SSDI and other insurance plans.

• Must possess a valid Class D driver's license and have acceptable driving record.
