About the Role

As the Program Manager of Transitions, you will play a pivotal role in shaping the operational excellence and strategic direction of Meridian. You will:

• Supervise and coordinate service delivery including program planning, administration, implementation, development, and evaluation of programs and services for a particular facility

• Oversee management of facility and program, including client care and services, staffing, internal and external program requirements, program activities, and facility management

• Supervise professional and paraprofessional staff and staff activities and perform related supervisory duties

• Monitor progress toward program goals and objectives

• Develop and implement individual client treatment plans

• Monitor records and documentation required for the daily operations of the facility and programs

• Serve as consultant to facility staff and assist in the development of appropriate programs and treatment plans for difficult clients

• Serve as program and client advocate, consultant, and liaison with community agencies to ensure continuity of care.

• Oversee treatment of clients with other counselors and day treatment coordinator, other programs of service, case manager, and family

• Prepare client, program, and administrative reports and forward to appropriate persons

• Oversee compliance of medical records, clinical paperwork and record keeping with all internal and external standards and requirements

• Performs other duties as required to meet program and departmental goals and objectives.

What We're Looking For

• A Bachelor's degree from an accredited college or university with major coursework in psychology, counseling, social work, rehabilitation or a human service field

• An Associate's degree from an accredited college or university with major coursework in psychology, counseling, social work or rehabilitation or a human service field, and two years direct mental health related experience
