This includes, but is not limited to, Advance Directive issues, patient/family advocacy and coordination of community resources, while maintaining professional social work standards.ESSENTIAL FUNCTIONS
- Conforms to the National Association of Social Workers (NASW) Code of Ethics
- Assists patients and families with psychosocial needs including counseling as needed and coordination of family meetings
- Recognizes problems, systematically gathers data, identifies/understands underlying issues, synthesizes complex issues, seeks input from others and makes difficult decisions to formulate appropriate treatment plans.
- Engages and collaborates with patients, families, multi-disciplinary staff and the community in care planning, delivery and discharge. Builds solid relationships, negotiates effectively and works well with individuals of all ages, backgrounds and socioeconomic position.
- Provides on-call services for Hospital, Physician clinics, and inpatient units.
- Advocates for patients and helps them navigate the health care delivery system.
- Functions as the liaison between patient, physician, adoptive agency or private attorney and patient accounts and as a support person for the mother placing her baby for adoption, both prior to and following delivery.
- Bachelor in Social Work required, Masters in Social Work preferred
- Current Iowa License preferred
- Experience: a. One to three years professional social work experience preferred. Previous experience in acute care preferred.
