Opportunity
- Join and collaborate with a palliative care physician, and other palliative care team members, in an active practice to deliver palliative care and related health care services to patients and families.
- Perform bedside palliative care patient/family evaluation.
- Assist with diagnosis, treatment, and management of acute and chronic health conditions.
- Provide palliative care services in both inpatient and outpatient settings, including in clinics and/or through home visits.
- Consult with palliative care physician or designees as needed, inform primary physician of services provided and collaborate with other physicians as needed.
- Assist in all facets of care coordination for palliative care referrals.
- Hours are M-F 8:00am – 5:00pm
- No call expectations initially, but a call component may be added in the future
- Graduate of accredited NP program
- Current WI RN and APNP license
- Current DEA
- Other certifications as required
- Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred.
