Overview:
Management for the assigned inpatient Care Management population. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Qualifications:
Education
Degree in Psychology, Sociology, Science or related field, Minimum required
Experience
2 years in a clinical area, Minimum required
Other
Strong verbal and written communication and facilitation skills. Strong assessment skills, time management skills, critical thinking skills. Advanced computer skills. Is competent to care for patient age groups including neonatal/infant, child, adolescent, adult and geriatric adult patients. Skills and knowledge of human behavior and clinical aspects of medical Social Work. Must have ability to interact effectively with a variety of people and situations at all levels of the organization.
