Provides case management for patients and families to enhance their psychosocial functioning. Performs duties and responsibilities in a manner consistent with mission, values and Mercy Service Standards.Position Details:
Education: Master's degree required in Social Work, Counseling or other human service related field. Licensure: Experience: Minimum one year Behavioral Health experience preferred. Certifications: Other: Preferred Education: Preferred Licensure: Licensed Master's Social Worker (LMSW), Provisional Licensed Professional Counselor (PLPC) or eligibility for licensure preferred. Preferred Experience: Preferred Certifications: Preferred Other: Co-worker must be able to communicate orally, in writing and demonstrate proficient computer skills.
