The Manager of Care Management is responsible for the direction, leadership and oversight management for the assigned Care Management departments. This position is designed to facilitate an effective process of the Mercy Care Management Model supporting quality patient care, patient safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; collaborating with facilities to include leadership coordination as evidenced by metrics, optimizing performance and adoption of best practice.

Position Details:

Education: Associates Degree in Nursing is required. Bachelor's of Science in Nursing is preferred.
Licensure: RN (Registered Nurse) in state of practice is required.

Experience: 3 years of clinical experience in case management or related field is required

Certification/Registration: BLS (CPR) at hire date, required, or within 90 days of hire. Certification in Case Management, Preferred
