A Care Navigator is a key member of the FullWell Care Management team that will support the RN care manager(s) and help outreach to patients following ED visits, in-patient hospitalization, and stable disease management patients. This role will coordinate with the patient’s primary care provider for medication refills, post- hospitalization appointments, and any RN delegated duties, etc. This role requires a strong telephonic communicator who can speak to patients to encourage/schedule preventive screenings, coordinate with primary care practices to schedule visits, assist patients with their care needs, and partner with their assigned RN care manager. We are looking for someone with initiative who works well with our Care Management team to ensure excellent quality patient care with great outcomes.

Pay Range
$35.17 - $56.10 /hour

- Utilize your clinical knowledge and critical thinking to assess patient symptoms and determine the most suitable sports medicine team member for their care (e.g., physician, physical therapist, athletic trainer).
- Directly schedule patient appointments into EPIC, ensuring a seamless and efficient process
