This role supports individuals in navigating complex systems, addressing social determinants of health, and improving access to care across assigned counties. Navigators support implementation of Hub-approved community health initiatives and collaborate with local partners to improve community health outcomes.

Position Details
Minimum Qualifications
- Associate degree required; Bachelor’s preferred in Social Work, Public Health, Community Health, Human Services, or related field.
- Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
- Strong interpersonal, communication, and problem-solving skills.
- Ability to work independently in community settings with limited supervision.
- Experience working in rural or underserved communities.
- Knowledge of social determinants of health and community-based resources.
- Completion of a Community Health Worker training program (such as Partners in Care) preferred.
- Ability to complete approved Community Health Worker training during the first year of employment preferred.
