Job Summary
Join our dynamic healthcare team as a Home Health Registered Nurse serving the vibrant community of St. Tammany Parish! Duties
- Conduct comprehensive patient assessments to evaluate health status, including vital signs, physical examinations.
- Develop and implement individualized care plans aligned with patient needs and medical directives.
- Administer medications, including injections and IV infusions, following safety protocols.
- Perform wound care procedures such as suturing, ostomy management, and dressing changes to promote healing.
- Manage complex conditions like acute pain, infectious diseases, memory care, and chronic illnesses within the home setting.
- Educate patients and families on disease management, medication administration, nutrition, and safety measures to foster self-care.
- Document all patient interactions accurately using Wellsky/Kinnser systems while adhering to HIPAA regulations.
- Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate comprehensive care.
Requirements
- Valid RN (Registered Nurse) license in Louisiana with current CPR/BLS certification. Valid drivers license.
- Med surge experience is a plus.
- Strong knowledge of medical terminology.
- Home Health experience along with Wellsky/Kinnser is highly desirable.
- Ability to perform advanced skills such as catheterization, laboratory specimen collection/processing.
- Excellent communication skills for patient education and interdisciplinary collaboration; comfort working with diverse populations including geriatrics (memory/dementia care), and individuals with disabilities.
