A Discharge Coordinatorwho excels in this role:
Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge.
Creates and maintains up to date aftercare coordination community resources.
Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients’ provider and therapist.
Provides resource information to patients and families as appropriate for future care needs.
Actively communicates with clients, families, aftercare providers, and referral sources.
Demonstrates proactive communication with those involved with the patients’ treatment.

What we’re looking for
Applicants should have a Bachelor’s degree in counseling, social work, or related clinical field from an accredited school. Additional requirements include:
Counseling or social work license preferred
Three years of discharge coordination experience in a healthcare setting preferred
One year of experience working in a psychiatric facility preferred
Must exhibit a comprehensive understanding of HIPAA
CPR certification is required within 30 days of employment and prior to any patient contact
De-escalation certification is required within 30 days of employment and prior to any patient contact
