How you’ll contribute

A Discharge Coordinatorwho excels in this role:

  • Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge.

  • Creates and maintains up to date aftercare coordination community resources.

  • Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients’ provider and therapist.

  • Provides resource information to patients and families as appropriate for future care needs.

  • Actively communicates with clients, families, aftercare providers, and referral sources.

  • Demonstrates proactive communication with those involved with the patients’ treatment.

What we’re looking for

Applicants should have a Bachelor’s degree in counseling, social work, or related clinical field from an accredited school. Additional requirements include:

  • Counseling or social work license preferred

  • Three years of discharge coordination experience in a healthcare setting preferred

  • One year of experience working in a psychiatric facility preferred

  • Must exhibit a comprehensive understanding of HIPAA

  • CPR certification is required within 30 days of employment and prior to any patient contact

  • De-escalation certification is required within 30 days of employment and prior to any patient contact
