As a member of the MERCI team, you'll play a pivotal role in our Discovery Program, reaching out to individuals who, due to medical fragility, are unable to access our on-site campus regularly. In this role, you'll provide personalized 1:1 support, assisting with daily living activities such as feeding and personal care. You'll be integral in implementing tailored care plans, ensuring adherence to dietary and medical needs, and fostering a proactive, positive environment where participants thrive.
What You'll Do:
- Work closely with individuals in the Discovery Program, providing 1:1 support in their homes.
- Assist participants in integrating into the community, enabling them to engage in work, volunteer opportunities, and social activities.
- Respect individual preferences and rights, allowing participants to shape their own schedules and activities.
- Support participants in taking ownership of their services and support, empowering them to lead fulfilling lives.
- Foster successful community integration while recognizing when to step back appropriately.
- Maintain accurate records and reports.
- Attend team meetings, training sessions, and in-service programs.
What You Need:
- High school diploma or equivalent.
- Proficiency in English communication, both verbal and written.
- First Aid and CPR certification (training provided if needed).
- Ability to lift up to 50 pounds.
- Successful completion of health check, including TB test (costs covered).
- Clear background check, including fingerprinting.
- Valid California Driver's License and insurance.
- Reliable vehicle.
