Position Details
Under the direction of the Blended Case Management Supervisor, you will:
- Serve as the primary contact for up to 30 individuals in the community
- Develop, coordinate, and implement personalized service plans
- Provide case management, service planning, and coordination with other agency providers
- Offer supportive counseling to help individuals achieve their goals
- Work alongside county reentry grants to support individuals transitioning from jail back into the community
- Participate in on-call responsibilities as needed

Qualifications:
The ideal candidate will possess the following qualifications:
- High school diploma or GED AND AT LEAST 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science required.
- 2 years’ experience in public or private human service, with 1 year being direct care required. Experience must be with clients over the age of 18.
- Valid Driver's license required.
- All clearances required by OMHSAS regulations.

- Bachelor's degree in sociology, social welfare, psychology, gerontology, or other human services.
