- Perform therapy sessions in private homes and senior communities
- Document patient progress accurately within EMR systems
- Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging-in-place strategies
- Support telehealth/virtual care when appropriate
