Role: In Home Healthcare LVN

Responsibilities

  • Provide high-acuity nursing care to pediatric and adult patients in their homes
  • Conduct skilled assessments, administer medications and treatments
  • Perform ventilator and tracheostomy care
  • Monitor complex conditions closely
  • Collaborate with RNs, therapists, and families to implement care plans
  • Ensure patient safety and educate caregivers

Qualifications & Skills

  • Licensed Vocational Nurse (LVN) license
  • Experience with high-acuity pediatric and adult care preferred
  • Strong assessment, clinical, and communication skills
  • Ability to work independently and as part of a team

Key Duties: Deliver comprehensive home health nursing, support patient safety, and promote ongoing caregiver education within a compassionate, patient-first environment.
