Overview
We are seeking a compassionate and dedicated Certified Home Health Aide to provide exceptional in-home care to clients across various settings. Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, including vital signs and overall condition
- Administer medications and document medication administration accurately using Electronic Health Record (EHR) systems
- Support patients with specialized needs including Alzheimer’s care, dementia care, hospice care, and memory care
- Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely
- Assist with medical procedures like catheter care, wound care, and basic first aid as needed
- Prepare nutritious meals tailored to dietary restrictions and assist with feeding when necessary
- Perform light housekeeping tasks including laundry, cleaning, and maintaining a safe environment
- Document all patient interactions and medical records in compliance with HIPAA regulations
- Collaborate effectively with healthcare professionals using EMR systems for seamless patient care coordination
- Provide emotional support and companionship to promote mental well-being
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated experience working with individuals with developmental disabilities or disabilities in general
- Knowledge of medical terminology relevant to home health caregiving
- Experience administering medications and managing medical documentation accurately
- Familiarity with long-term care practices, hospice services, dementia, Alzheimer’s care, and memory support strategies
- Certification as a Home Health Aide (HHA) is required; CPR certification is strongly preferred
- Experience working with EMR/EHR systems for documentation purposes
- Ability to handle sensitive situations with compassion while maintaining confidentiality under HIPAA guidelines
- Skills in patient observation, caregiving for seniors or disabled individuals, and providing comprehensive home & community care
