About the Role
- As a Physical Therapist, you’ll deliver skilled treatment plans across a continuum of care—including outpatient rehab, skilled nursing, and assisted living settings. Whether you’re a seasoned PT or a recent graduate, you’ll find the mentorship, support, and collaboration you need to grow your career and make a lasting impact.
Key Responsibilities
- Initiate and implement personalized physical therapy programs based on physician orders
- Maintain detailed evaluations, progress notes, and discharge summaries
- Collaborate with nursing and rehab staff to enhance residents//' functional outcomes
- Contribute to interdisciplinary care planning meetings
- Maintain treatment documentation, track patient statistics, and request necessary supplies
- Participate in or lead staff training and education
- Provide leadership support in the absence of the Director of Physical Therapy
What We’re Looking For
- Graduate of an accredited Physical Therapy program
- Active NY State Physical Therapy license
- Strong interest in working with geriatric, subacute, and adult patient populations
- Previous experience preferred (but not required—new grads are encouraged to apply!)
- Leadership qualities and ability to direct support staff when needed
