As a part of our Maui Case Management Team, you'll be at the heart of our mission—providing recovery-focused support to individuals with psychiatric disabilities. You'll help navigate challenging situations and emotional conflicts, making a meaningful difference in the lives of those we serve.
Case Manager Requirements:
- High school diploma or equivalent required for some positions; bachelor’s degree required for others.
- Bachelor’s degree in behavioral sciences (e.g., psychology, social work, human services, etc.) strongly preferred.
- 1½ years of specialized training or experience in human services or case management preferred.
- Basic computer literacy required.
- Competency or willingness to learn how to support individuals with dual diagnoses.
- Patience, flexibility, and strong interpersonal communication skills.
- Ability to manage emotional or crisis situations with professionalism and empathy.
- Must pass a criminal background check, pre-employment drug screening, and possess a current TB clearance or TB X-ray.

Maui Case Manager Responsibilities:
- Assessment & Service Planning: Conduct behavioral health assessments and develop person-centered treatment plans in alignment with contract timelines.
- Recovery-Oriented Support: Empower individuals to live self-directed lives, supporting them with budgeting, appointments, entitlements, and skills to enhance independence.
- Service Coordination: Collaborate with medical staff, other case managers, and community partners to ensure effective, holistic care.
- Clinical Documentation: Maintain accurate, timely, and professional records in compliance with MHK standards and policies.
- Hospital Discharge Support: Engage consumers within 7 days post-psychiatric hospitalization and help reduce the risk of re-hospitalization.
- Team Collaboration & Supervision: Participate in clinical supervision and training to continue growing professionally while enhancing service effectiveness.
- Community Outreach: Provide follow-up and outreach services, including after-hours coverage as assigned.
- Adaptability: Respond to changes in consumer needs or agency priorities with flexibility and a solutions-focused mindset.
