Key Responsibilities
- Perform comprehensive assessments during Start of Care visits, evaluations, re-certifications, discharges, and supervisory visits.
- Develop and modify care plans in partnership with physicians and patients, recommending resource utilization to achieve optimal outcomes.
- Make clinical recommendations based on patient assessments and implement protocols aligned with diagnoses.
- Educate patients, families, and caregivers on disease management, community resources, and self-care techniques.
- Respond to care plan changes, determining if significant condition changes require OASIS documentation.
- Apply nursing processes effectively, demonstrating proficiency in communication, assessment, and problem-solving skills.
- Registered Nurse (RN) license in the practicing state.
- Preferred 1-3 years of experience in home health or hospice care.
- Strong communication, assessment, and clinical decision-making abilities.
- Ability to work independently and as part of a team, maintaining confidentiality and adhering to safety standards.
