RN Clinical Supervisor

Position: Clinical Supervisor (RN) / Clinical Case Manager

Role Overview: This leadership position combines clinical expertise and administrative duties, managing a caseload of medically fragile patients and ensuring high-quality care delivery. Key responsibilities include conducting "Start of Care" assessments, reviewing physician orders, updating care plans, and validating nurse competencies. The role involves mentoring field staff, assisting with hospital discharges, and collaborating with operations and business teams to ensure compliance with all policies and regulations.

Responsibilities

• Manage internal casework and evaluate nursing staff development
• Perform assessments to transition patients from facilities to home care
• Review and update care plans and documentation
• Interview, train, and validate clinical staff
• Enhance field staff's clinical skills for safe, high-quality nursing care
• Collaborate with cross-functional teams to support operations and growth
• Ensure adherence to state, federal, local, and organizational policies
  
  

Qualifications

• Current RN license in the practicing state
• Minimum of 1 year of hands-on nursing experience
• Ability to travel within the designated territory
• Reliable transportation, valid driver’s license, and pass MVR check
• Current CPR certification (AHA or ARC)
• Willingness to supervise in-home skilled nursing care across age groups
