Collaborate with interdisciplinary teams to ensure seamless, patient-centered care, promote positive interactions, and uphold quality standards.
Qualifications & Skills: Registered Nurse (RN) required; Bachelor's degree preferred. Minimum of 2 years in Case Management or 3 years clinical nursing experience. Familiarity with psychosocial assessments, discharge planning, and InterQual preferred. Strong communication, coordination, and compassionate patient interaction skills.
